The board of directors of the Georgia Lacrosse Officials Association (GLOA) has determined that every GLOA member official, in order to be eligible for game assignments MUST read and sign the following declaration. You can sign electronically by checking the “Ready” box on this main page of the Georgia Lacrosse Officials Association’s entry page of the Arbiter. The “Ready to be Assigned” box is found at the top right hand side of the front page of the Arbitersports web page under the MAIN tab after you log in. This will constitute a legal electronic signature. Waiver follows.
I understand that my working relationship with GLOA is that of an independent contractor, and as such may choose to accept or decline officiating opportunities. I agree that I will give preference to assignments from the approved GLOA assigner(s) before accepting any other games at a similar level. I authorize GLOA to provide necessary tax related information to the IRS, other tax authorities or appropriate lacrosse organizations for which my services have been provided. Furthermore, I agree the Georgia Lacrosse Officials Association, its’ Board of Directors, its’ designee’s, Assigners, and Trainers shall not be liable, and agree to release them, from any and all liability for any injury or damage sustained as a result of any assignment. I understand and agree that this wavier is for one year only. Further, I understand that agreeing to this wavier does not carry any obligation on the part of the Georgia Lacrosse Officials Association or the Assigner for a specific number, location, level or type of game assignments. I understand that any assignment is subject to change or cancellation due to changes by the teams, other circumstances, or in the discretion of the Assigner(s). I also agree to keep the Assigner(s) fully informed of all closed dates and times via the Arbitersports website and of any conflicts of interest I may have with any team or program. I have also been provided access to a copy of the association’s By-Laws and Code of Ethics on the association website and have read and understand these documents.
